Beginning this spring, Carondelet
Health Network is making the transition from
a paper-intensive medication reconciliation
process to an electronic process. This new
process will improve patient safety by
providing physicians with a streamlined
medication reconciliation workflow and more
accurate, up-to-date medication profiles.
The go-live dates are as follows:
- St. Joseph’s Hospital: March 15th
- St. Mary’s Hospital: May 3rd
- Holy Cross Hospital: June 21st
Credentialed Physicians, NPs,
and PAs are required to complete a 1 hour
training session prior to go-live date.
A physician will be unable to
transfer/discharge patients if training is
not completed by go-live.
Training is available in the following
forms:
Classroom
Training: Please see the Training
Calendar for your hospital and register for
a class by calling the IS Physician Line at
419-6264 or emailing Nicole at
nleblanc@carondelet.org.
Practice/Group
Training: If your practice or
group would like Carondelet Trainers to hold
a Group Training Session at your
own facility for your multiple physicians,
please contact the IS Physician Line at
419-6264 to schedule training.
Department
Meetings: Please speak with your
Department Head to arrange a 1-hour Training
Session at a hospital based department
meeting. Training can be scheduled by
calling the IS Physician Line at 419-6264.
